OUTREACH Physical and Occupational Therapy and Speech Rehabilitation PLLC (“OUTREACH Rehabilitation”) may collect mobile phone numbers and related contact information from patients, authorized caregivers, referral sources, and other individuals who communicate with our practice.

Mobile phone numbers may be used to send text messages (SMS) related to our services, including appointment scheduling and confirmations, scheduling updates, referral follow-up, therapy service coordination, and responses to inquiries.

Mobile phone numbers, text messaging consent, and related information will not be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes.

Information may be shared with service providers that assist OUTREACH Rehabilitation in providing text messaging and other operational services, as necessary to provide those services and subject to applicable privacy and confidentiality requirements.

Consent to receive text messages is voluntary and is not a condition of receiving services. Message and data rates may apply. Message frequency may vary. Recipients may reply STOP to opt out of future text messages.
