Clinical Director, Occupational Therapy Department
Danielle Conklin leads Outreach Rehab’s Occupational Therapy team with a mix of clinical expertise, warmth, and a deep love for working with older adults in their homes. As Clinical Director, she mentors fellow OTs, supports thoughtful, personalized care, and still enjoys being hands-on with the patients who inspire her daily.
She started her career in a skilled nursing facility and has spent the past five years bringing care directly into people’s homes—where therapy often feels more personal and impactful. For four of those years, she served as a Senior OT, guiding new therapists and helping patients find confidence and safety in their everyday routines.
Danielle earned her Master’s in Occupational Therapy from Columbia University and a BA in Psychology from SUNY New Paltz. She’s currently pursuing a specialization in geriatrics to deepen her work with aging adults.
When she’s not working, you’ll find her running, hiking, exploring new restaurants, or spending time with friends and family. Danielle’s path to OT was fueled by her passion for helping people live fuller, more independent lives—and that passion hasn’t wavered.

Clinical Director, Speech-Language Pathology Department
Nancy Buckley leads the Speech Therapy team at Outreach Rehab with a calm, grounded approach and a deep commitment to helping patients maintain their voice, independence, and connection to others. As Clinical Director, she collaborates with a team of SLPs, oversees quality care delivery, and believes that connection starts with communication.
With six years of experience, Nancy specializes in working with adults and older adults across home care, assisted living, and skilled nursing. She treats a wide range of conditions including stroke, Parkinson’s, dementia, and head and neck cancer, and is certified in both LSVT LOUD and PhORTE. Her clinical focus includes communication and swallowing disorders such as dysarthria, aphasia, and dysphagia.
Nancy earned her Master’s in Speech-Language Pathology from Montclair State University and her Bachelor’s from Fordham University. She also leads group communication sessions, helping patients build confidence in real-life social settings.
Outside of work, Nancy enjoys traveling, baking, crocheting, and exploring new cuisines. Her passion for speech therapy comes from a belief that communication is one of the most human—and most powerful—parts of care.

Director of Physical Therapy
Peoni Amin leads the Physical Therapy team at Outreach Rehab with a calm, steady presence and a deep commitment to patient-centered care. As Director, she mentors clinicians, supports high-quality therapy in the home, and fosters a collaborative, growth-oriented team culture.
With over seven years of experience—including two in outpatient care and five providing in-home therapy—Peoni continues to treat patients herself. She believes the best care happens when therapists are present, practical, and deeply engaged in their patients’ goals.
Peoni earned her Doctor of Physical Therapy from Arcadia University, a Master’s in Exercise Science from SUNY Buffalo, and a Bachelor’s in Physiotherapy from Sardar Patel University in India. She is currently pursuing a specialization in geriatrics to deepen her expertise in serving older adults at home.
Known for her thoughtful leadership and collaborative approach, Peoni brings both clinical insight and heart to everything she does. Outside of work, she enjoys reading, hiking, and traveling—and is continually inspired by the resilience of the patients she serves.
